If you have a project that can positively impact the oral health of residents in nursing homes or other care facilities, GDG would like to consider funding it.
We accept requests for project funding in the first quarter of each year. Projects are approved each April. To be considered in April, requests must be postmarked by March 31.
Any one project must not exceed $10,000.
To submit your project, please complete these PDF forms and send them to GDG. The first 2 pages of the form are the project request and instructions. The second two pages are the reporting forms for the completed project and instructions.
Approved projects must submit receipts for reimbursement or invoices for payment to the Projects Committee. The committee will forward the receipts and invoices to the Executive Director who will write the checks promptly. Project personnel may not incur liabilities in the name of Geriatric Dental Group (GDG) or Special Delivery Dental Care, Inc. (SDDC). Project personnel will not represent themselves as representatives of GDG or SDDC without the written permission of the Executive Director and the Projects Committee.
